As a part of your child’s requirements for school, a physical examination has been required for students in Prekindergarten or Kindergarten and in Grades 1, 3, 5, 7, 9, 11 and new entrants. A law was enacted in 2008 that expanded health screenings to include dental health examination of students in New York State.
When we require that your child have a physical examination, we will be requesting a dental certificate as well. Below under the Related Files Links, please find a dental health certificate for you to print out and take to your child’s dentist to be completed. Please return the form to the School Nurse as it will be filed in your child’s Cumulative Health Record.
If you do not have a regular dentist, you can access dental care through PRASAD by calling (845) 434-0376 or Hudson River Healthcare at (845) 791-7484. You may be eligible for free or reduced cost care that is based on family income.
