Hello - if you are new in the District, or are registering your children for the first time, you will need to have certain information with you when you begin the registration process. Located below are the forms that need to be completed. You can print out a complete packet of forms or individual ones. Please don't hesitate to contact the Office of the Registrar if you have any questions.
You will need to show the following:
- Legal proof of residency with your physical address, such as a lease, rental agreement, copy of school tax bill or a utility bill in your name with a recent date. If this documentation is not available, the District will consider other forms of documentation.
- If you have school records from previous schools please bring them. You should also have the complete name and address of prior schools.
- Up to date immunization records
- Proof of student’s age
- Proof of custody (if applicable)
All registration is conducted in the Office of the Registrar at District Central Offices in the Elementary School (845-482-4610 x 3006.)
